Inspection Form | Health and Safety Checklist for Childcare Centers

This document will help you ensure that your building and staff are in compliance with state regulations regarding childcare at all times. The document is editable, so you can customize to your state requirements and age-groups served.

At least once a day, preferably at the top of the morning, walk through your entire campus (inside and outside) and perform an inspection. Confirm that all state regulations are being met. Immediately correct any violations. You can randomly inspect your center multiple times a day using this checklist. The observer visits every classroom and other areas listed on the checklist. The age group and ratio for each observed classroom are circled at the top. The observer checks yes or no for all applicable items. If a particular item is not applicable, “N/A” is written in the note column. If a required item is not observed, “no” is checked. The observed teacher is required to fix the issues. Then “corrected” is written in the note column.

Classroom Inspection Checklist

• Visit each classroom and observe for a few minutes
• Complete the Health & Safety Checklist form as you observe each class.
• Fill in the date and teacher
• Circle the age group and correct ratio for the class. If mixed age, circle the age of the majority.
• Check Yes or No for all applicable items.  If not applicable, write N/A in the “Note” column
• If “No”, have the teacher make the adjustment. Then write “corrected” in the “Note” category.
