Individual Counseling Data Collection Spreadsheet for School Counselors

Use this Google Sheet to collect data on the students you see regularly for individual counseling! All you have to do is put in your student data and the form will automatically calculate averages, percent change, and generate graphs in a striking data report. With minimal time you can create professional reports for your stakeholders to advocate for your school counseling program. You don’t need to know how to do anything fancy in spreadsheets - I’ve already done all the coding for you!

This spreadsheet is not intended for detailed note-taking. It’s organized to track the impact your individual work with students makes so that you can report to your stakeholders. It allows you to review a pre and post survey as well as the effect your counseling has on objective school data such as attendance, GPA, discipline data, etc.

This download also includes all 36 ASCA Mindsets & Behaviors converted into positive statements that can be used on a survey with a likert scale (ex: B-SMS3: Independent work is converted to the statement, “I can work independently.”). The spreadsheet has an assessment generator which allows you to choose up to 3 M&B that you are using with a student and it creates a printable survey for you! The spreadsheet also allows you to use your own survey if you prefer.

Limitations of this spreadsheet:

• The spreadsheet collects data for up to 250 students. It’s not intended for you to track every student on your caseload - just those that you see regularly for counseling.
• This is a single use license for use by one counselor or individual to collect data on the students they meet with. If other counselors or individuals at your school or district plan to use it, please purchase additional licenses at a discount.
• Pre and post assessments may target up to 3 mindsets & behaviors using a likert scale.
• Collects and reports data for up to 3 types of outcome data: attendance, academics, and discipline referrals.
