As a special education teacher to secondary aged students, a major focus of my teaching involves writing meaningful Transition Plans for students' IEPs. These Transition Plans center around their success outside of the classroom and help prepare students to start thinking about where they see themselves in a vocational environment.

Since Transition Plans are all about the student, it is is very important to make the process revolve around them as well. I have created this interactive notebook to address this very need to bring students' voices to the center of these very powerful conversations. With more and more districts going toward person centered planning, this print and go download is the perfect fit to get students involved in the IEP process!

Because a Transition Plan is needed for ALL students who turn 16 years old, this is a great investment for your classroom. It includes the major components of a solid Transition Plan, and helps walk students through the process of understanding its purpose in a meaningful fashion. Although this download was initially designed to support special education teachers with students 16 and older, ANY special education teacher looking to get students more involved in IEP meetings could benefit from this very student-friendly approach.

How I Use This In My Own Classroom:

I prepare several blank notebooks to have on hand in the beginning of the year as IEPs come up. As they do, I set aside time and work with students individually to help fill each section out. We discuss what the areas mean and help them brainstorm good qualities to put down for each one. After they are finished, students are now aware of, not only their potential vocational path in the future, they are also able to summarize IEP goals they will be working on this year as well. I then bring this notebook to the IEP and have students present each section with their family and IEP team.
