No longer is it necessary for SLP's to have to dig through filing cabinets to locate several months' worth of progress notes to average progress for quarterly grading periods!! This interactive excel form was created to maintain monthly averages for each IEP goal & objective addressed. See at a glance a student's current progress on goals as compared to the Mastery Criterion Level on their IEP. Data can be presented to parents easily at conferences. OT's and PT's may find this useful as well.
