This is a great tool for guided reading. It is a great way to keep up with groups and what each group is doing.
This product has really helped me to stay organized with my guided reading lessons. I put these into a binder so they are all in one place and easy to access.

This Guided Reading system is quick, easy and VERY handy to help you plan and keep documentation of your Guided Reading Groups, as well as give immediate feedback and goals to your students. It gives you data and proof of what you have been working on and shows growth of individual students. You could also use this during intervention and bring with you to RTI meetings, meetings with administration, and parent/conference meetings.
I have made this editable so you can now type directly on the templates. You can add and take away as many text boxes as you need. Hope this helps!
Guided Reading is so individualized that it is daunting to remember exactly what you did with each individual student as well as the progress he or she made. Even when you work with one group, you are hitting multiple skills within that group. This template takes care of those issues!

*front-to-back template for your Guided Reading groups
*running-record sheet with fluency calculation formula
*blank template for you to customize
The front of the page is for you to prepare your plan, determine the skill(s) to be worked on, the book you will use, and the group with which you’ll use the plan. There is also room for vocabulary and word work. There are choices of activities to do with the group before, during and after the reading.
The back of the page is for you to list the individual members of the group. You have room to put the skill, goals, any notes you take while he or she is reading, and the feedback you give the student. I also included blank pages for you to customize to better fit your needs.
