Group Therapy Data Sheet

I developed this double-sided group data sheet for use with my speech therapy students, but it can be used for all subject areas including occupational therapy, physical therapy, reading intervention, math intervention, etc. It is designed to be used with groups of up to five students.

On the top of the data sheet there are spaces to write in session dates and times, student names, and goals. Below the goal section, there are spaces to record dates, student names, lesson objectives, data, and comments for nine group sessions. I find it far less cumbersome than shuffling multiple data sheets in the small spaces in which I work with students. Having the IEP goals easily accessible also makes planning for subsequent sessions easier.
