Goal Planning for Staff Google Form

This Google Form is customizable and provides clear prompts to help staff create and submit goals. The purpose of this form is for school leaders to help gather and analyze the data to inform professional development. This form provides data trends for the staff as a whole and individual data. When using this form, you may find it helpful to use the data in the responses tab as well as create a linked Google Sheet to view the data in a different format.

With a few edits to introduction and first question, you can also adapt this goal planning form to be used with students or other populations.
