Get Out of Jail Free Card/Bathroom Pass

This is a student card given to students on the first day of school. It allows the students 7 exits for the semester. The card explains that it is used for all exit strategies, including bathroom, phone call, nurses, etc. You will need to switch my name for your name, but I included it to keep the formatting correct. At the end of the semester, I give 5 bonus points for each hole that is left unpunched. I use a simple handheld hole punch that I keep at my desk. This allows students to simply walk up. I punch it, and they carry it out as their pass. I laminate them before giving to students and print them on the goldish-yellow paper like Monopoly cards.
