End of Year Parent Survey

This end of the year survey allows parents to anonymously provide feedback on positive and negative aspect of the school year. This form allows you to seek input from parents on how they feel you are doing as a teacher. It emphasizes how feedback from them is the most important and allows you to become a better teacher. Send it home with each student near the end of the year and find out what you are doing well and how you can improve!
