Description

Science experiments, icebreakers, games- lots of engaging labs and activities can involve food/candy. If you are planning to use food in activities, then this digital and printable food allergy permission slip can help keep your students safe! This resource includes an editable PowerPoint form, a printable PDF with a form field for the deadline, and an editable Google Form.

Food can be a fun way to make lessons more engaging. However, there have been times when I was surprised to discover that some of my students had dietary restrictions, were intolerant to certain foods, and/or had food allergies. Typically this information is available in student records, but there have been times when it wasn't!

Additionally, some parent(s) and/or guardian(s) may not want their children to participate in food related activities for other reasons. It's best to play it safe and get (documented) permission first!

Please note:

  • This resource comes in a zipped folder
  • You must have a Google account to access the digital version of this resource
  • Due to the font artist's TOU, the PDF is not editable.
  • The text on the PowerPoint version is editable. However, due to the clip art TOU, the frame and banner are not editable.

Tips/Suggestions for Use:

  • Students may need reminders to return their slips on time. Be sure they have written it in their planners/agendas (if they have one)

  • The Google Form is a convenient way to get parent/guardian permission without having to track down paper forms (let alone make tons of copies of them)

  • If you prefer paper copies, then I suggest keeping extra forms on hand for any new students you may get throughout the year

  • It can be helpful to give these slips out during the beginning of the school year. Having them available during Open House (if your site is having them in person) can be a quick way to get them filled out.
