EDITABLE SLP Paperwork Binder

Get organized this year! This EDITABLE Paperwork Binder includes data collection, IEP/screening/referral trackers, contact logs, a weekly schedule, and CEU and attendance trackers for you to edit, print, and place in a binder.

Note: The fonts are embedded so anything you type in will match with the fonts in the document. Due to copyright regulations, the backgrounds and fonts themselves are NOT able to be changed and will NOT be available for use in other documents (you have to download the fonts to your computer for that). You will need to download Adobe Acrobat Reader (it's free) to open and use the binder.

For all editable forms: text boxes can be typed into, or you can print the forms and fill everything in by hand.

Binder covers: 5 different designs options to personalize your binder. Type in your name, school year, or any other details you want!

Data Collection section with 2 cover options; personalize the text using the text boxes

• Daily Data Collection Logs : Includes space for goals and IEP info for 1 student. Type in the student info (name, goals, IEP date, etc) and print! 3 border options. *2 of each page so you can easily print front and back!*

Medicaid section with 2 cover options; personalize the text using the text boxes

• Medicaid/Medical Assistance- compatible Data Collection Logs: Include signature line and NPI/license number line for MA compatibility. Type in your NPI/license once and they will auto-populate to all of the NPI lines! 3 border options. *2 of each page to easily print front and back!*

Communication section with 2 cover options

• Parent notes: Includes check boxes with common goals/activities as well as lines to write other notes. One page each (2 half-sheet notes per page) for Articulation, Language, and Fluency. 2 border options (one "brighter" and one plain, for older students). Editable signature line with cursive text and Reminder/Notes section- type into either of these and the info will auto-populate to both pages in the set!
• Contact Logs: 1 individual contact log with editable header for contact information, and 1 general contact log
• Services Info: Includes blanks for 2 providers with contact info, schedules, and goals for each; a list of provider types to circle; and a place to write Strategies/Cues that work. Great for leaving with teachers and daycare workers! 3 border options.

Caseload Management section includes 2 cover options

• Progress Report Tracker : Boxes for 4 dates at the top, with 2 columns under each date so you can note when each report is done AND when it is printed/given to parent so none slip through the cracks! Type names & dates right into the form. Digital checkboxes are included.
• IEP Due Dates by Month : includes 4 months on one sheet (Sept-Dec, Jan-April, March-August) , so you can see at a glance when your IEP's are due.
• IEP Due Dates with Blank month : use for each month

Eligibility section includes 2 cover options

• Evaluation Trackers: allows you to write in where you are in evaluations to keep due dates from sneaking up on you. 2 pages of 20 lines each
• Screening Logs : keep track of screenings. 4 pages with 25 numbered lines each (1-100), plus one without numbers.
• Referral Logs: keep track of referrals.
• Weekly Schedule: write times down the lefthand column and add your sessions in for each day (in pencil, because you know you're going to be changing it a billion times!)
• Blank section pages : have a section of your binder that I didn't include? Create your own cover page! Fonts will match the fonts on the other section covers.

Included in separate files in resource:

• EDITABLE Attendance tracker: Excel file that includes rows for 60 students. You can include all students on one page or have one for each site you visit. Weekend days are grayed out for ease of use. 2 pages (30 rows each) for each month from July 2018-July 2019, updated every year.
