Dysphagia Assessment Report Template

I've developed this tool during my clinical experiences in Speech Pathology to complete swallowing reports efficiently by simply filling in each of the blanks and copy+pasting relevant information.

This goes into more depth than I've seen used by other clinicians but it's designed so that you have the foundations for completing the assessment efficiently and you can leave out the parts you see irrelevant.

Yellow highlight has been used to draw attention to sections which will need to be amended to suit your client. To remove the yellow, set the Paint Fill and Highlight to 'No Colour'.

Text has been colour coded according to the clients presentation

Green - appropriate/normal

Orange - Mild/weak impairment

Red - Severe impairment/unable to asses

Dark red - additional information
