Need a simple way to check-in with your students while apart? Using Google forms and some simple, daily questions is a great place to start. This product is Google-ready and will instantly be ready for you in your Drive.
I post one form per day and I even record a quick video of myself answering the "this or that" question for the day. I use these forms to not only stay in touch with students, but to keep an eye on how my students are feeling and dealing with their distance learning experience. You could space these out to once per week or use them any way you see fit for your students!
WHAT DOES THE FORM CONTAIN?
Each form contains the same, editable questions. You may change anything you'd like -- they're completely yours! The forms contain the following fields:
BASIC INFO:
-Today's Date
-Student name
-Class period
HOW YA DOIN?
-This or that? (a photo-based question)
-Here, feel free to leave me a note. How are you feeling? What's going on? What questions do you have about class? (open-ended)
-I'M NOT OKAY: I'm having a really difficult time...
- This does not apply to me
- I need to schedule a Zoom meeting with you
- Can you please call?
- I just need you to know today was rough. No need to talk about it, just wanted you to know.
- Other...
