Description

I created this form to use with my 2nd grade students. Previously students had to "walk laps" as punishment at recess. This proved to be an ineffective consequence for many. Now my cohorts and I rotate hosting Recess Detention and Recess. Students fill out this to document behaviors. These forms can be filed in each student's personal file or kept in a notebook depending on how you personally want to manage it. I keep a notebook full of blank forms on one side of a divider, and I place filled out forms on the other. I file the paperwork into individual forms later. No one has access to this notebook except my cohorts and admin. Students check which behaviors landed them in detention and write explanations as to why the behavior(s) is unacceptable and what they can do differently next time.
