Description
This data collection sheet provides a quick and easy way to take data during your therapy session. There is room for up to 5 goals per student, or 1 goal for 5 students in a group (such as an artic group). Each goal has up to 10 dates of entry, so you can record data for the full grading period, for 10 consecutive sessions, or for 10 random dates. There are 10 data entry points, so you can quickly count the percent correct (example, 8 out of 10 correct, or 80%). There is also a section to add comments for each date recorded, so you can record notes about the activity, the types of prompts needed, etc.
SCORING: There is a scoring legend at the bottom of the sheet to account for amount of cueing or prompting needed. Of course, you can modify the scoring to your own needs.
INSTRUCTIONS: Simply write in the goal in the "Goal" box, write the date, and record your data in a quick, efficient way. The boxes are small to allow for discrete recording for those students who try to peek at your notes. You can clip the forms onto a clipboard and flip through to view each child's goals during the therapy session.
NOTES PAGE: There is a notes page on the back (when printed double-sided). You can add session notes or comments on the back of the form or on a second sheet of paper. The single-page form and the two-page form are included in the zip file. (Thanks to Lesli Phelan for the suggestion!)
