Description
This single-page behavior report provides an easy way to communicate student behavior to parents. At the top of the page is a spot for a daily notation (stamp, sticker, color, number, whatever system you use) and parent initials. This is a quick way to let parents know whether it was a good or bad day.
At the end of the week, you can fill out the bottom half of the report where you rate students in 5 areas:
- following directions
- kindness to others
- self control
- using time wisely
- doing quality work
This gives parents a more detailed picture of their child's behavior that week.
Several versions of the behavior report are provided:
- Version A has weekly ratings of Excellent, Satisfactory, and Improvement Needed.
- Version B has Meeting Expectations or Improvement Needed with added space for comments.
There is also a choice in wording at the top of the page. One says "I Can Follow Classroom Rules" at the top. The other uses the word "Expectations" instead of rules.
