What educators are saying

I love this resource! There are different options which help to ensure I am using the correct log to meet what is needed per student. Additionally, the smiley faces make it easy for my self-contained special education students to understand how their day is going!
This chart was super helpful when creating a behavior contract for a parent meeting! Thank you for a wonderful resource!

Description

Are you looking for a no-prep, easy to use way to track a student's behavior? Look no further than these editable behavior charts! Perfect for data collection, parent communication, student goal setting and more! With 3 different formats of daily and weekly charts, there is a behavior chart that will fit every student's needs.

The first format allows you to write the subjects for the day. Then, you can rate the student's behavior for that subject.

The second format is categorized into morning and afternoon. Then, you can rate the student's behavior for each portion of the day.

The third format has each day of the week listed. You rate the student's behavior for each day of the week. Note that this format has to be printed two-sided. Monday-Wednesday are on the first side and Thursday-Friday are on the second side.

Each format has three different rating types:

  • The first rating includes graphics of a smiley face, straight face and a sad face.
  • The second rating includes a thumbs up, sideways thumb and a thumbs down.
  • The third rating includes the numbers 3, 2, 1.

Each report card allows for you to rate the student's behavior and to write comments. There is also a space for a parent signature.

Because all 3 formats of the behavior report card have 3 rating types, there are 9 behavior report cards in all!
