Consultation Log - Documentation Form for Related Services

This document from The Organizing OT is an easy to use and efficient means of recording contact with staff when providing consultative supports. The form includes space to record date/time, the means of communication (whether it is face-to-face, via email or phone), and the contact person. It's a great way to quickly log the collaborative time that speech therapists, physical therapists, and occupational therapist spend working with regular and special education teachers!
