Record session notes and track CF ASHA Hours at the same time!
Before your submit your finalized session notes for direct contact hours, where do you write out your notes? Some CFs enter their notes directly into their billing system or their respective Dept. of Ed. system. Some draft them out first. Use this template to do that --and count your CF hours at the same time. If you have a preference for working in GOOGLE instead of Excel, this sheet can help you stay organized!
Summary Tab
--enter direct and indirect CF requirement to start counting hours
--e.g., 80% of 1,260 for direct and 20% indirect
Record your session notes AND count your hours too
--enter the activity (e.g., Direct 1 = direct hour spent in segment 1, etc.)
--use the form to track session notes (then copy final note into your appropriate system)
--record the time spent in each SESSION or... count DAILY or even WEEKLY hours
