Classroom Sign-out Sheet

Keep track of students who leave your room with this sign-out sheet. I was constantly frustrated with students who used the restroom too often and I wanted an easy way to limit the daily or weekly times they could leave.

This is a .pdf file. It was created using Pages for Mac and if you would like the .pages file, let me know. I can also export it to Word, however, I will have to check if it exports correctly.
