Description
This is a from that I give my students at the end of the school year to evaluate my classroom and teaching. This allows them to tell me what assignments they liked and did not like. I love doing this as the end of the year to adjust for the next school year. The students also like this because they are able to give their opinion and allow their voice to be heard.
