Great resource for kids to tell more about themselves. I used this as a planning sheet for them to create their own identity poster.
I loved using this as an opening of the year exercise with my fifth and sixth graders. It was a great starting point for connecting identity to how they see the world.
Description
Use this handy form as an activity to identify aspects of one's identity such as looks, personality, heritage/race, gender, family structure, interests, where you live, religion/beliefs, etc.
Use for individual reflection as a tool to understand identity or as a class activity. Students enjoy when you gather the forms and read a couple unique circles of the student's identity to see if the class can guess the student.
