CELF-5® Screener Informal Summary Template

This Word document provides you with an informal report template with easy ways to quickly and efficiently individualize and prepare screening results for the CELF-5 screener (ages 5-8 and 9-21) and share the results with the classroom teacher(s) and/or Instructional Support Team Members.

I often screen many children in my building and created a summary to provide to the classroom teacher(s) and/or my building level support team. Please note that since this is an informal summary of the student's performance, the language utilize is not standard report writing language, so that school related colleagues can better understand how the student performed.

This form should be used as a template and modified based on individual student performance.

If you are using this template for formal documentation purposes, then you might need to make some changes and adjust the language and format accordingly.

How to use this template.....

Administer the screener to the student in accordance with The CELF-5 Examiner’s manual and edit the sample report with information accurate to the student
