Make outlining, writing and publishing a book report easier for you and your students with this book report template for 5th grade! This book report template for 5th grade (3rd and 4th grade too) is meant to be used in a multitude of ways.

Use just a few sheets, like the Mini Book Report page or collecting quotes page to assess if students are able to summarize a book correctly and support with evidence from the text.
