Behavioral Observation Recording Form (Interval Recording)

This Excel form is based on the BOSS assessment (Shapiro, 2013) and allows the user to record student on-task and off-task behavior in a systematic way. The percentage of on-task and off-task behavior is automatically recorded at the end of the observation period. Operational definitions are included and can be modified as needed. I highly recommend using an interval timing app (Interval Timer is my personal favorite lately!) with this form to make sure the interval recording is done properly.
