These editable contract systems consist of 4 different booklets that can be easily mass-produced. All 4 booklets allow you to:
~ Set measurable behavior goals
~ Designate specific "small" and "large" rewards
~ State exactly what criteria the student needs to meet to earn their rewards
~ Give students feedback for positive behaviors
~ Collect ABC data needed to conduct a Functional Behavior Assessment (FBA)
There are four booklets that are designed to meet a variety of needs at different developmental levels. The booklets are half-sheets, so each set you print makes two booklets. Contents:
Booklet #1 (recommended for PreK or early elementary students)
~ Cover Page (students can personalize by writing in their name and drawing a self-portrait)
~ Behavior Goals - Picture Form
~ Rewards Menu - Picture Form
~ "Smile, You're Doing Great!" Behavior Tracking Form - teacher draws a smiley face each time the student engages in one of their goal behaviors
~ Behavior Incident Recording Form - allows you to document inappropriate behaviors along with information about antecedents and consequences
Booklet #2 (recommended for early elementary students)
~ Cover Page
~ Behavior Goals - Picture Form
~ Rewards Menu - Picture Form
~ "Check It Out!" Behavior Tracking Form - teacher draws a check mark each time the student engages in one of their goal behaviors
~ Behavior Incident Recording Form
Booklet #3 (recommended for mid to late elementary students)

~ Behavior Goals - Write-In form
~ Rewards Menu - Write-In form
~ "Tally It Up" Behavior Tracking Form - teacher makes a tally mark each time the student demonstrates one of their goal behaviors

Booklet #4 (recommended for late elementary or middle school students)

~ Behavior Goals - Write-In forms (1 page for each goal)
~ Rewards Menu - Write-In form
~ "Behavior Bank" Behavior Tracking Form - teacher makes a tally mark in the "Credits" box each time the student demonstrates one of their goal behaviors, and makes a tally mark in the "Debits" box each time the student engages in inappropriate behavior
~ Behavior Incident Recording Form
As you may have noticed, some of the pages from the booklets are the same - they have been arranged into four different sequences so that they are ready to print in a pinch, and so they can be mass-produced to distribute for Tier 2 and Tier 3 interventions. You can always choose to mix & match pages across booklets. You also have the option to add or substitute any of the following:
~ Mini-behavior intervention plans (2 options)
~ Condensed Write-In Behavior Goals page (all 3 goals on 1 page)
~ Write-In Behavior Incident Recording form (the form included in all the booklets has a checklist format)
All the forms referenced above are included in a separate PowerPoint file that allows you to edit the text however you'd like. Please note that you need PowerPoint 2013 to edit, and that you may need to download a few custom fonts from Kimberly Geswein (they're free for personal &classroom use!) to get the same "look" as the forms in the .pdf file.
