This is a short introduction letter for related services providers to send home to parents. It is applicable for occupational therapists (OT), physical therapists (PT), and speech therapists (SLP), as well as hearing, mobility and orientation, and other disciplines. It includes a space for sharing scheduled therapy times, as well as therapist contact information. Additionally, it asks parents to provide some additional information to help therapists best understand and serve their children. It is a great starting place for constructive and cooperative dialogue with families and school-based therapists!
