Bathroom Chart

This is a great pdf that I created to help with bathroom and toilet training documentation in my self-contained classroom for students with special needs. This can be used with any age student (I personally have used this with kindergarten through high school aged-- older students typically are in adult briefs at that point) This pdf is very simple to use-- there is space for 4 students per page. Each student has a small table for the week (M-F), with room for 4 separate toilet trackings. Each time a student uses the bathroom (either in the restroom or in their pull-up), staff can document the date, time, and description of the incident (bowel movement or urination). I have my staff (teacher or paraprofessionals) initial in each box-- sometimes can be very insightful to look for patterns. This chart has been very useful in communicating with parents about their child's toileting routine and pattern throughout the school day.
