Health Room | Nurse Pass
A nurse pass or health room pass is required at my elementary school. This product has many options for nurse pass templates. There are full pages, half pages, and quarter pages. These passes have been a game changer to communicate with the nurse and health room staff! There are also options of color or black and white.
How to use
Print the template of your choice. If you want to skip writing your name on every pass, then write your name on the passes and then make copies. That way you only fill in the student's name, date, and injury/illness.
This could also hold students accountable for what they tell their teacher versus what they say when they get to the nurse's office.
Student Contact Information
This download includes a student contact/parent information sheet. Every year I feel like I miss out getting someone's email, address or how they get home on the first day of school versus the rest of the year. This sheet has everything in one place. I am all about organization!
Student Name Tags
This resource contains many options for name tags or name plates. There are options with a number line, hundred chart, colors, analog clock, coins, right/left hands, and place value with base ten blocks.
There is a space to write student names and they include many other concepts that could be helpful for students.
How to Use:
- Choose your template of choice
- Write student's name on the template (or they could write their name depending on grade level)
- Cut and laminate for longer-lasting name tags or name plates
Staggered Start Labels
Does your school have a staggered start the first few days of school? When I taught pre-kindergarten and kindergarten, dismissal was when I felt so much anxiety about sending kids home. Often, transportation can be confusing, especially if parents switch.
This product helps cut down on my stress level at the end of the day when it is time to send kiddos home.
