Articulation Baseline Data and Progress Monitoring

This document will allow you to collect baseline data for the following articulation sounds:

F
V
S
Z
Sh
Ch
Voiceless Th
Voiced Th
L
R
It provides ten words for each of the initial, medial, and final positions of words. There is an area to record the date and the percentage for that dates testing. There are four more slots following the baseline testing slot to allow for quarterly progress monitoring throughout the year. There is also a section to test the students production of sounds in sentences. Pictures are not provided. I provide the student with verbal cues for the word (e.g. you get in this at night to get clean before bed = Bath) but you are welcome to find your own pictures. Hope you find this document helpful in gathering baseline data and tracking progress following the gathering of baseline data.
