Have you been asked to write a letter to your student's doctor, but do not know what to write?
This download contains 3 templates including
1. ADHD (Attention-deficit/hyperactivity disorder) and ADD letter template with symptoms that you can adapt to your own student.
2. Social Emotional Letter for withdrawn student who may need psychiatric or psychological assistance.
3. ASD (Autism Spectrum Disorder) letter for family doctor or physician. Again, you can adapt the observations as needed
