AAC Data Collection Sheet

This a session-based AAC data collection sheet that includes a QR Code that links to a video on how to use it. You can take a wide range of data on your AAC user's communication, such as your focus goal (e.g., social skills, linguistic skills), your teaching targets (e.g., teaching LIKE and MORE), what prompts were used (e.g., pointer cues, verbal cues), how the student performed, what other messages were made, what were the purposes of the student's communication (e.g., protesting, commenting), and what other methods of communication (e.g., vocalizations, reaching).

There is a place at the bottom of the sheet to reflect on what worked well and to jot down ideas for your next session.
