AAC Assessment Data Collection Form

This is an assessment tool that I created several years ago to help perform AAC evaluations. This helps to support a comprehensive assessment and provides areas to record assessment information in the following domains:

-> Access

-> Symbol Understanding

-> Audio Imitation

-> Usage

-> Maintenance/Ownership

-> Responsivity to Aided Language Modeling

-> Literacy

-> Speech Intelligibility

-> Sign Language

This tool, when combined with additional techniques such as interview, survey, and observation helps to support the ability of evaluation teams to identify the necessary information to determine AAC needs.

**Please note that due to the highly individualized nature of AAC assessments, this product is only a data sheet with cuing and yes/no questions. You will still need to gather materials to run many of the sections of this assessment.
