    PARENT LETTER TEMPLATES

    Create your personalized letter template using Form 1. This document is an editable letter introducing the band program and your campus. It provides places to fill in your contact information as well. Listed are 6 positive benefits explaining why students should join the band program. A separate template example is also included for your review. 

    Form 2 is an editable document that communicates the instrument selection results with parents. You can include information and dates explaining how parents can acquire an instrument. Included is a separate template example.

    Form 3 is a fillable generic parent/guardian permission form. This is a great resource to begin dialog between you and your parents. Parents can complete this form and return by way of their computer. Parents will check the "YES, NO, or UNDECIDED" box to confirm. Parents can now leave feedback, questions, or concerns for your review. 

    PERSONALITY SURVEY

    Go paperless with a newly added fillable form! This is a helpful tool designed to break the ice as you conduct your instrument petting zoo with students! Link students with instruments using their personality traits! In this questionnaire students will be grouped into 6 personality categories that are characteristic to each instrument. Group 1: Flute, Group 2: Double Reeds/Horn, Group 3: Single Reeds, Group 4: Trumpet, Group 5: Low Brass, and Group 6: Percussion. 

    During the instrument selection process there are times that students score really well on several instruments. This tool can help narrow down the instrument selection process. This questionnaire has 27 fun and engaging questions. Attached is a matrix that lists core personality traits of the 6 instrument groups. 

    The director would read the student responses and determine which personality trait is most applicable by checking the appropriate box at the bottom of the sheet. Each box is numbered according to the 6 instrument groups. If the personality trait cannot be determined, the director would select the 7th box for that student. Be sure to tell students not to work below the STOP sign. For optimum use, record this data before conducting your instrument selection tests.

    STUDENT PREFERENCE CARD

    Go paperless with a newly added fillable form! Brief and concise students will check the "YES" box if they are interested in band. Students not interested will check the "NO" box. If students have an option to take band along with another elective, they should check the "YES" box as well. Students can select (3) of their favorite instruments on the form. Save paper and copy front to back onto card stock if using hardcopies.

    INSTRUMENT TESTING RUBRIC

    Go paperless with a newly added fillable form! Looking for an instrument testing rubric? Look no further! This tool provides a simplistic point system for a smooth instrument selection experience for both student and director. 

    It is recommended that students select (3) of their favorite instruments before using the rubric. For optimum use, students should also complete a Personality Survey prior to testing.

    INSTRUMENT PETTING ZOO POSTERS

    Make your instrument petting zoo easy, colorful, and engaging! Posters are arranged in instrument families; woodwind, brass, and percussion. Included in the collection is a separate “Join the Band” poster crafted to grab the attention of the students while recruiting.
