504 Plan Self Advocacy Sheet

If you are a school counselor, special education teacher, school psychologist, admin, or parent (or anyone else working with students!) wanting to prepare a student with a 504 Plan to better understand their support plan for their transition to college (or just for their own knowledge), this is for you!

Use this Support Services Self-Advocacy Sheet to go through their 504 Plan with them to prepare to advocate for themselves to talk to their teachers or for a meeting with their future college's disability/accessibility support services center. This sheet helps them identify what their disability is, organize their documentation, highlight their areas of need, and provide a list of accommodations that have helped them (and can hopefully still get in college).
