Who Am I? - personal information sheet survey homeroom cumulative phone log

Students can tell you about themselves and it has a phone log on the back for keeping track of phone calls home and notes on that student.

Students tell about family, hobbies, sports, favorite foods, expectations, etc. They enjoy pairing and sharing about their answers also.
