Through my own practice, I've noticed that many clients feel intimidated by the stark, assessment-like aesthetic of the various forms I'm continually handing them, so I've designed these PHQ-2, PHQ-9 and GAD-7 handouts as a means of making the assessment process more welcoming to the client. These designs maintain the specifics of the original published assessments, however appear with different graphics, to cater to a wider range of clients' needs/understandings.

-The GAD-7 (Generalized Anxiety Disorder 7-Item Scale) assesses symptom severity associated with general stress and anxiety, as well as specific to the four most common anxiety disorders (Generalized Anxiety Disorder, Panic Disorder, Social Phobia and Post Traumatic Stress Disorder). This is a widely accepted screening tool that many clinicians have integrated into their weekly practice, in order to more effectively assess client symptoms, and to gain a more comprehensive understanding of how the individual client is affected by anxiety.

-The PHQ-2 (Patient Health Questionnaire - 2) inquires about and assesses the frequency of depressed mood, with a score ranging from 0 to 6. Individuals self-report on two questions, and this screen is deemed a "first-step" approach; individuals who screen positive require further evaluation to determine whether criteria for depressive disorder have been met.

-The PHQ-9 (Patient Health Questionnaire - 9) is a multi-purpose tool used for screening, diagnosing, measuring, and monitoring the severity of an individual's depression. The assessment is comprised of 9 questions according to DSM criteria and allows patients to self-report on a rating scale of "0" (not at all) to "3" (nearly every day). This module helps clinicians to accurately assess client symptoms, in order to gain a more comprehensive understanding of how the individual client is affected by depression symptoms.
